  • Determine & discuss the stage of change.
  • Reasons for smoking (WHY test).
  • Nicotine Dependence (Fagerstrom Test).
  • Offer information re: pharmacotherapy options.
  • Set a quit date.
  • Review quitting history.
  • Review potential challenges and triggers.
  • Encourage support of family and friends.
